Data Privacy Consent

To assist in coordinating and facilitating your medical care, we need to collect, record, and store your data. This data may include personal and sensitive personal information. In compliance with the Data Privacy Act of 2012 of the Philippines and the General Data Protection Regulation of 2016 of the European Union, we need your permission to process your personal data for this purpose.

What is personal data?
Personal data is a collective term referring to all types of personal information. It may be: Personal information, or those which can be used to identify you. This may include your name, address, telephone number, etc.
Sensitive personal information include those that may be related to any of the following: racial or ethnic origin, age, marital status, political opinions, religious beliefs, health, sexuality or sex life, biometrics, government-issued numbers and/or licenses.

What is “processing” of my personal data?
“Processing” refers to any operation performed on personal data, including collection, recording, organization, storage, updating, modification, retrieval, consultation, use, consolidation, blocking, erasure, or destruction.

How will you use my personal data?
Your personal data will be used for coordinating and facilitating your medical care including the choice of hospitals, medical specialists and diagnostic facilities. Also, it will be used for quality improvement, patient safety, risk management, health research and education, as well as to generate statistical data relevant to Shiphealth’s operations.

Will you share my personal and sensitive personal information? Who will you share it with?
We will only share your information with your permission, and only for the purposes we have stated above (unless required to do so by law). For these stated purposes, your information will be available to Shiphealth and its authorized employees, its affiliates and service providers. It will be shared with your employer, manning agency, insurance provider, Protection and Indemnity club and its correspondents and third-party service providers. Any of which may be located outside of the Philippines.

Where will my personal data be stored?
Your personal data will be stored in an electronic database system accessed by authorized personnel. Paper copies of your data may also be stored securely in the office premises and may be accessed by authorized personnel who coordinates and facilitates your medical care.

How long with you store my personal data?
The Personal Information and Sensitive Personal Information you provide will be retained by Shiphealth for a minimum period of [10 years] (“Personal Data Retention Period”), or as long as necessary for the purpose of maintaining your medical records and to comply with applicable laws, rules and regulations on retention of data. Unless retention for a longer period is required for reasonable cause, we will dispose of your Personal Information upon the lapse of the Personal Data Retention Period in accordance with applicable laws and regulations.

Can I withhold my consent?
Yes, but we may not be able to coordinate and facilitate your medical care.

May I obtain the information you hold about me?
Yes, you have a right to ask for a copy of your information and to correct any inaccuracies.
